New Patient & Client Information
Download the New Patient & Client form below and fill out the requested information. Please arrive 15-20 minutes prior to your first visit to allow for time to enter you and your companions information into our computer. If we are seeing your companion for a second opinion or as a referral, please bring a copy of the medical records and x-rays.
All dogs and cats must be on a leash or in a carrier.
Maze Animal Hospital excepts cash, check, Visa, MasterCard, Discover, American Express, and CareCredit as forms of payment for services.
